Referrals

If you speak with your physician, orthodontist, dentist, etc. about a referral to Speech Pathways, they can fax an order to us at 712-340-1516, and we will reach out to you!

We are more than happy to help you through the process of getting a referral. If you’re having any trouble, please contact us, and we’ll walk you through the next step.

Frequently Asked Questions

  • Give us a call for a free phone consultation to determine if a speech and/or feeding evaluation may be beneficial. You can also talk to your physician about sending us a referral, and once we receive it, we will give you a call to set up the initial visit.

  • We accept Blue Cross Blue Shield, Iowa Medicaid and MCOs (Iowa Total Care, Wellpoint, and Molina), Medicare part B, Cigna, HealthPartners, and Medica. We are an out-of-network provider with UnitedHealthcare. We also accept private pay. We will reach out to your insurance after the initial evaluation in order to determine what out of pocket costs you may have per your insurance plan.

  • We will bill your insurance directly. Once your insurance claim is processed, if you have a balance, we will send you an invoice through our Client Portal. If you have a co-pay, we do request that be made at each session via check or AutoPay.

  • We accept HSA/credit/debit card through our Client Portal system. You have the option to login and pay as you go or sign up for AutoPay. We also accept checks.

  • Yes. A formal evaluation is always completed at the first session. This helps us determine whether or not services are warranted, discuss goals, and develop a plan of care moving forward. It is also required by most insurance companies.

  • You do need a referral from a physician per insurance guidelines. If your physician agrees that an evaluation is warranted, they can fax a referral, including a facesheet, to Speech Pathways at 712-340-1516.

    If you don't have a referral but want to discuss your concerns, please give us a call. After our phone consultation, we can decide if an evaluation is warranted and discuss getting a referral if so.

  • We work with all ages- newborns to geriatrics.

  • It depends entirely on the needs of each patient. There is no one size fits all. We will discuss goals, recommendations, and possibly estimated length of therapy if we are able to determine that at evaluation or throughout therapy.

  • Absolutely! We love collaborating with school SLPs in order to ensure your child/loved one is getting consistency and progressing with their speech and/or feeding therapy goals.

  • We will complete a formal, standardized assessment as well as informal assessment. If they qualify within those assessments, yes, we can still work with them.

  • This depends entirely on each patient. Some patients are appropriate for 1-2x/week, while others benefit from 3-4x/week. We will discuss all of this at the evaluation and make recommendations there. We are also aware that schedules are busy and although we may recommend 2x/week, if you are only able to do 1x/week, we will work around that.
